The design of a systems model for comprehensive service delivery to youth, aged 0-20 years, and their families is presented.
Abstract
A volunteer task force, convened by the City-County Youth Bureau of Syracuse-Onondaga County, N.Y., developed the model. The model is a client-centered system of service delivery that combines the most appropriate services from public and private sectors with a holistic approach to meeting the needs of youth and their families. It emphasizes immediate response to requests, whether for information, crisis intervention, or long-term service. Three major components compose the model: preliminary stages concerned with intake and assessment, the creation of the service plan for those desiring more extensive service, and a component concerned with service delivery and case management. The preliminary stages (1 and 2) determine the nature of service delivery (information, crisis intervention, interim support, and/or service plan) and establish the basis on which a service plan may develop. In stage 3, the youth and his/her family must decide whether to commit themselves to developing and following through on a service plan. Stage 4 involves the development of the service plan, with the involvement of the youth, his/her family, and all others significantly involved with the youth. Stages 5 and 6 involve the implementation of the plan: service delivery, case management, and follow-up to determine client satisfaction and effectiveness of the service plan. The data collection process is briefly described. Five references are provided.
