Abstract
THE BULK OF THIS DOCUMENT IS COMPOSED OF QUESTIONNAIRES AND INTERVIEW FORMS USED IN A SURVEY WHICH ATTEMPTED TO DETERMINE THE DEPTH, QUALITY, AND RETRIEVABILITY OF CURRENT ACCIDENT AND INJURY RECORDS. THE FORMS COVER SEVERAL KEY SAFETY-RELATED PERSONNEL PRACTICES AND SAFETY PROGRAM MANAGEMENT ITEMS SUCH AS THE DESIGNATION OF SAFETY RESPONSIBILITY WITHIN A DEPARTMENT, FREQUENCY OF SAFETY DISCUSSIONS WITH THE CHIEF, EXISTENCE OF A SAFETY BUDGET, AND TRAINING PRACTICES.
