This article includes various sample forms used by a British police surgeon in the performance of his duties; brief explanations of the forms are provided.
Abstract
One sample consent form is for use when explicit written consent to a medical examination and the provision of samples for forensic purposes is required from a patient. A form for use in the event of sudden death provides for information pertinent to the handling of the body subsequent to the examination as well as information relevant to the decision on whether to conduct a criminal investigation into the death. A sample referral letter is for use in referring patients to other medical personnel; it includes space for information on medical history and findings as well as treatment. A record of sexual offenses specimens lists the possible evidence samples obtained during the examination and provides a space for the specimen number. Another sample form is designed to assist police surgeons in conforming to the 1973 British regulations for the misuse of drugs. It is the form provided to police and contains information on the drugs to which the patient is addicted and the drugs prescribed. Other forms are for use in responding to a court subpoena, in informing rape victims about their health care, and in informing appropriate parties about an examination conducted to determine time of death.
