THE APPENDICES CONTAIN COPIES OF THE VARIOUS QUESTIONNAIRES SENT OUT TO AGENCIES WITHIN THE CRIMINAL JUSTICE SYSTEM OF THE THREE COUNTIES IN ORDER TO GENERATE BACKGROUND MATERIAL FOR THE STUDY. FOR A SYNOPSIS OF THE ENTIRE PROJECT SEE NCJ-009048.
