This guidance is a companion document to the Overdose Fatality Review (OFR) Data System Version 2.0 Codebook. This version of the guidance was released in February 2022 and is an updated version that was created, based on Version 1.0 pilot user feedback. The guidance is divided into four sections: Overdose Fatality Review Case Information, which includes 20 subsections such as OFR administration, decedent demographic information, drugs at the scene of death, interventions following overdose, trauma history, criminal justice history, social service, education history, case-specific recommendation summary, and a narrative section, among others; County Profile; County Health Rankings; and OFR Next-of-Kin.
