This logbook is designed to record correctional staff visits to restricted units within a facility.
Abstract
Proper documentation can avoid the correctional litigation often associated with the segregation unit and the conditions of confinement existing in these units. The standards of the American Correctional Association require that staff regularly visit, inspect, and evaluate the unit. Written policy, procedure, and practice regarding the segregation unit require regular fire and safety inspections; daily visits from the senior correctional supervisor, qualified health care official, and members of the program staff; and maintenance of a permanent log containing information on each inmate admitted to the segregation unit including name, number, housing location, date admitted, type of infraction, tentative release date, and special medical or psychiatric problems. This log should also be used to record all visits by officials to the unit. Each entry consists of the month and day of the visit, name and title of the visitor, general nature of the visit, and exact time in and out.
