This report describes a project designed to review and modify the recommended forms and case record procedures of New York City's Probation Case Record System. The appended forms that were reviewed and adopted as well as the adopted casefolder structure form the bulk of the report.
Abstract
The project developed 72 standard case record forms for departmental use, a standardized casefolder for use in all departmental branches, and standardized index cards for all branches and established a forms manager to control forms inventory and proliferation. Costs savings to the department as a result of the simplified procedures and more concise records appear to be considerable. A written procedures manual for use by all departmental personnel is needed. A suggested procedures manual is described and illustrated.
