The experience of the San Bernardino County Sheriff's Department in implementing an over-the-counter medication program for its inmates is discussed.
Abstract
The San Bernardino County Sheriff's Department began an over-the-counter medication program for inmates in January 1993. The purposes of the program are to teach inmates to be more informed and responsible health care consumers, to more effectively utilize health care staff by reducing sick calls for minor ailments and to reduce pharmacy costs. Among the issues that need to be considered before such a program is implemented is the need to involve all facility staff from the beginning to ensure that safety and security are not compromised. Inmates need to be educated regarding symptoms of common ailments, prevention, comfort measures and when their symptoms are serious enough to require professional attention. Providing adequate access to over-the-counter medication is another important consideration. A system for allowing indigent inmates access without allowing for abuse needs to be devised. Finally, once implemented, the program must be evaluated to see if cost savings are being realized and resources allocated better without jeopardizing patient health. A summary of the success of San Bernardino County's program during its first 6 months of operation is included.
